WeCare Home Health LLC is a home health agency incorporated in December 2016. We are a Medicare-accredited Home Health Agency licensed by the State of California, by the California Department of Public Health (CDPH) and certified by the Accreditation Commission for Health Care (ACHC)

What we Offer
Our Services
Skilled Nurses
Our nurses will work with your doctor and other health professionals to provide quality home care services. Their assessment, skilled care, treatment, education, support and collaboration with other disciplines will help our patients reach their maximum potential.
Physical Therapy
Physical Therapists will come to your home to provide strengthening exercises, gait training, mobility training, safety teaching, and instructions on the use of durable medical equipments, among others.
Speech Therapy
Our Speech Therapists will help patients improve speech, assess and treat swallowing issues and assist patients with memory or cognitive deficits.
Occupational Therapy
Occupational Therapists help people of all ages to improve their ability to perform tasks of daily living (i.e. dressing, cooking and eating).
Home Health Aide
Our Home Health Aide Services assist with bathing, hygiene and grooming needs.
Medical Social Worker
Medical Social Worker assists patients and their family with referral to community resources
