Services We Provide
Medical Social Work services and assistance We provide services to the greater Sacramento area and in the nearby counties that includes Placer, Yolo, Yuba, El Dorado, Solano, Amador, Sutter counties and parts of San Joaquin and Contra Costa counties. We provide innovative and reliable care to patients at home who have en discharged from in-patient facilities or referred from MD clinics for a myriad of reasons not limited to: wound care, medication management, IV infusion and therapy, health teaching, caregiver training, respite care, continuation of rehabilitation therapy at home.
We also provide social services to assist patients who may need community referrals or services and home health aides to assist them with their personal and grooming and hygiene needs. Assists in the admission process of
the patient to the Agency by
performing an initial evaluation
Skilled Nursing
Registered Nurse coordinates
total patient care by conducting
comprehensive health evaluation
Home Health Aide
Follows the plan of care
to provide safe and competent
care to the patient.
